			<description>&#60;p&#62;Apologies.  The 22 forms have returned.  All the fields for the 22 forms are now visible in the form editor.  I just have to re-enter the shortcode onto the posts and pages and all works perfectly.&#60;/p&#62;

			<description>&#60;p&#62;Just called my hosting provider and repaired the corrupted database.  I can now create and save new forms which is good news.  But the original 22 still return the &#34;Oops! We could not locate your form&#34; error message, although they are still all listed in the form editor.  Can these forms be easily retrieved somehow or is it going to be simpler to recreate them all?  Unfortunately being the slow learner i am i did not export a backup of these forms.

			<description>&#60;p&#62;There are lots of things that can cause corruption in a database.  Power fluctuation, power outage, bug, randomness, hardware failure, improper shut down, etc.  Sort of like when a hard drive fails on a PC.  Most of the time it can be fixed easily, but it pays to involve the host so they can be sure it does not happen again.

&#60;p&#62;The MySQL database table is corrupt and needs to be repaired.  You should contact your host for support on how to proceed.  You might be able to repair the table using SQL commands in a tool like phpMyAdmin, but you should contact your host to be sure.  You may need to restore this database table from a backup if the repair fails.
