The HTML field you add to the form is not part of this submission: it's there for display. If you want, you can add that same text to a hidden field in your form. That way, it will be included in the submission and entry.

I would like to be able to output the HTML form fields in both the notification to the user and the administrator. We two forms that have "disclosure/disclaimer" information that the user reads and "signs". This text needs to stay intact with the form.

The link is http://www2.reveregas.com/application-for-employment/ the HTML that needs to stay with the employment information is located on the last page of the form.

Thanks in advance for your help to resolve this,

Lucy
