I have responded via email with the solution. The issue you have product fields with a placeholder value as the first value and the price for this placeholder is set to $0.00. You need to leave the price field completely blank. $0.00 is a valid value, a product can be no cost. A blank price, however, means there is no product. So you need to leave the price field completely empty for placeholders in Product drop downs.

And on a side note. What ever I try I cannot get the Phone Number field to accept any kind of style other than what shows.

Final Page Displaying Totals in a very odd way. I have tried everything, what am I missing?

Try it at the link above (Nothing will be charged, it's a demo form) to see what I mean.

Input is appreciated.
