This is really close to answering all of my questions. </p> <p>So on this dev page, I could add a bunch of regular form questions before the "Another Item<br /> Price: $1.75 Quantity:____" and then a few more questions after that, and change the button name from "Buy Now" to "Register Now" and upon clicking they would be sent to PayPal.
