&#60;p&#62;It's two months late but I've released a plugin to achieve this. Your client's problem was the same as mine so after I finished developing the code for my client I went ahead and released it as a plugin in the hopes it will help others. &#60;p&#62;But I already checked that one. I could see if I could modify this but the client doesn't want to spend much more time. He uses a lot of forms for internal &#38;amp; external communication.&#60;/p&#62;
&#60;p&#62;One thing i could do is say Godspeed to you guys, since the form business is a highly demanding one and i can see lots of business for the good/extensive ones.&#60;/p&#62;
&#60;p&#62;I will however keep an eye on your progress.&#60;/p&#62;

I was reading online Gravity Forms is the best but unfortunately  no PDF option. I checked addons on WP and there's one but i can't put a logo in there and change some specific layout in the output.&#60;/p&#62;
&#60;p&#62;I guess this is simply not possible at all?
