When I attempt to replicate what you describe I encounter no problems adding multiple Phone Fields to a form and editing each of them individually. If it is not, then it could be caused by code in your theme or another plugin causing issues with the JS used in the form editor. When I do so, it changes both field names to one or the other. It wont allow me to have a phone and fax number. I am using 1.6.2.1.1. Stephen
