			<description>&#60;p&#62;Hi,&#60;/p&#62;
&#60;p&#62;Hoping someone can assist. I basically need to know whether it's possible to build using some plugin in WordPress, a Pivot form as right now, I'm unsure if this is actually possible?&#60;/p&#62;
&#60;p&#62;I would like to build a form entry screen in WordPress where there will be say 5 questions down the left hand side of the form (vertically) but along the top (horizontally), I would like to display two weeks worth of dates, commencing Monday through to Sunday and then repeated again.&#60;/p&#62;
&#60;p&#62;So based on the above, each of the 5 questions answers, will be associated to two weeks worth of dates.&#60;/p&#62;
&#60;p&#62;Does anybody know whether this is actually possible in WordPress, using Gravity Forms or possibly performing the same end result but displayed differently?&#60;/p&#62;
&#60;p&#62;Hope the above makes sense.&#60;/p&#62;
&#60;p&#62;Any help would be much appreciated.&#60;/p&#62;
