I basically need to know whether it's possible to build using some plugin in WordPress, a Pivot form as right now, I'm unsure if this is actually possible?</p> <p>I would like to build a form entry screen in WordPress where there will be say 5 questions down the left hand side of the form (vertically) but along the top (horizontally), I would like to display two weeks worth of dates, commencing Monday through to Sunday and then repeated again.</p> <p>So based on the above, each of the 5 questions answers, will be associated to two weeks worth of dates.</p> <p>Does anybody know whether this is actually possible in WordPress, using Gravity Forms or possibly performing the same end result but displayed differently?</p> <p>Hope the above makes sense.</p> <p>Any help would be much appreciated.</p> <p>Thanks. </p>
