Can you use the shortcode in the default value, rather than the parameter box? The parameter box is use for a name which would be in the query string as shown above. I can't remember if the default value supports shortcodes or not. If it does not, we will need to use another way of pre-populating the form field. There are shortcodes for each of these, but I do not know how to pass that data to the Gravity form. I tried putting the shortcodes in the prepopulate parameter box, but that didn't work.
