Have you tried playing around and experimenting with the form builder yet? I'm a bit unclear on what exactly you are having trouble with. There's nothing crazy going on here with what you are trying to do. You should be able to do this with ease. Take a look at this documentation and play around with the form builder when you create a new form. All you have to do is click on the fields you want and they get added, so you build your form as you see fit.
