The form that I've built is now live on my website and works well. I'm amazed that I can build something so good - I'm very happy with Gravity. I'm looking at some refinements to make it as user friendly as possible.

			<description>&#60;p&#62;I have created a 9 page form for new patients to fill out online. Three of the pages have identical sets of questions about the health conditions that the people are coming to see me about. I would like to be able to have a radio button question at the bottom of the page of &#34;Health Condition 1&#34; page and &#34;Health Condition 2&#34; page giving people the option of jumping to the final page if they don't have a second or third health condition. My search of the forum hasn't come up with any solutions to this though it would seem to me to be a common scenario.
