			<description>&#60;p&#62;Damn! I didn't receive the notification of your reply...I just updated gravity forms and will check it out. You know what? I had different installations of gravity forms on different websites (different virtual hosts on the same dedicated server) and attachments from one form were mixed with entries belonging to another form on another domain..... Weird!

&#60;p&#62;there is one form submission, with the uploaded image and then a following form submission reports an uploaded image which is a copy of the previous image.&#60;/p&#62;
