They're the default form controls in the Mac version of Safari. If you want to style all of your forms to look like that, then you'll have to use some third-party script that replaces the defaults ( <a href="http://www.456bereastreet.com/lab/styling-form-controls-revisited/radio-button/" rel="nofollow">rendered differently in each browser</a> ) with images.
