			<description>&#60;p&#62;Need to create a form that allows a client to select which survey questions they would like to include in their mobile app.  In order for them to make a decision/selection, they need to see both the question and the answers.  Is there a way to list both questions and answers with a checkbox next to each question?  &#60;/p&#62;
&#60;p&#62;For example.  &#60;/p&#62;
&#60;p&#62;Question/Option 1:&#60;br /&#62;
How old are you?&#60;br /&#62;
a. 18-25&#60;br /&#62;
b. 25-30&#60;br /&#62;
c. 30-35&#60;/p&#62;
&#60;p&#62;The client would read through each question and answer and select the questions they would like to use in their mobile app...&#60;/p&#62;
