			<description>&#60;p&#62;You would not convert the PDF to a Gravity Form.  I would add form fields to your form to start (choose the most appropriate field type for each input) and then share the URL here and let us know what you want to change.  In most cases, you will be able to move the form elements around by using CSS which will be added to your theme's stylesheet.&#60;/p&#62;
&#60;p&#62;You may not be able to get it looking exactly like that PDF, but it should be close when you are finished.

&#60;p&#62;I want my form to be identical to or at least closely resemble this pdf form:&#60;br /&#62;

&#60;p&#62;which is basically a bunch of text with input text boxes laid out in a certain manner with a certain size font.&#60;/p&#62;
&#60;p&#62;Is this possible?  How would I go about making this?  Would I first try to convert this pdf form to html and then place the input text boxes?   &#60;/p&#62;
