That's why the form isn't being displayed properly.</p> <p>You can re-enable the conditional logic, walk through the themes header.php file, comment out the script references one by one, save and refresh the page to see if the form shows. I'm hoping someone will give me a clue as to where to start hacking at this. I know enough code to get myself in trouble but not enough to figure it out from scratch. </p>
