One was that the Yes/No field for "Would you like to tell a friend about us?" requires a unique answer. Remove that restrictions, otherwise you will get at the most one Yes and one No. That is not a place that should require a unique answer. </p> <p>The other problem was that I could not submit without checking the box "Yes, I would like to receive inventory updates!". Maybe that's how you intended this to work, but it's not intuitive. Maybe I don't want to receive updates, but I just want to tell a friend about your site? Maybe I already receive updates?</p> <p>In any event, the form worked on my site. If the form submits fine on your site (no validation errors) but the friend does not receive an email from you, then you have an issue with the notification emails being sent by your server.
