			<description>&#60;p&#62;Sure, you can do this by setting up individual coach forms. Or you can make one form with a drop down with the coaches info and based upon the selection send the notification to that coach via the routing option in the notification settings.

I have 5 coaches who all use the same questionnaire. The customers currently fill out the form manually, scan it and email it back to their coach. I need to automate this process and move the questionnaire online to my wordpress site. I want the customer to be able to fill out the form online and submit it to their coach. Does Gravity allow me to accomplish this? Maybe utilizing a dropdown list of coaches or the ability for the customer to add the coaches email? &#60;/p&#62;
&#60;p&#62;Either way I cant have it go to a centralized email account that is not monitored. &#60;/p&#62;
