When I asked for specifics she wrote this, "I filled out the form. The first time I tried to email it, I got a message that there are errors, and I saw where I had forgotten to put in my last name. I corrected the problem. Then I tried 2 more times, got the same error message, but there were no more errors indicated." I haven't a clue about how to troubleshoot. I don't have her entries captured in the back end so that function never triggered. This is not the first time I've has an occasional person say they have trouble with the form. I've upgraded forms. Suggestions? Ideas about what the issue is? </p>
