&#60;p&#62;I'm having a similar problem, not in debug mode, deactivated all of the plug-ins, set the theme to the default.  Still couldn't find issue.&#60;br /&#62;

&#60;p&#62;All the entry data gets collected including the 10 User Meta fields. The WP User account also gets created with out any problems.&#60;/p&#62;
