I'm doing a website for a printing company and their order form is long:&#60;br /&#62;

I'm setting it up here&#60;br /&#62;

on the new website. I'm using the Pagebuilder function in my theme which makes it easy to set up three columns...but to do that I'd need three different forms, one for each column and each with its own Submit button.&#60;br /&#62;
What is the best way to display this long form in two or three columns with one submit button?&#60;br /&#62;
