Adding "Middle Name" field to Name form

    It looks like this was also requested over two years ago, but for legal purposes, my form needs the full gambit: First, Middle, Last, Suffix.

    Any chance of more choices to the name field soon?

    I am not sure if that field will be changed any time soon, but for now you could just use input fields for each one, allowing you to collect that information.

  5. @alsc: If you use the Single Line field to capture the middle name, I know of no way to insert it directly in between the 'First' and 'Last' name fields. Additionally, if you use a separate Single Line Field for each part of the name (First, Middle, Last), and if the name is required, then an asterisk will show beside each part of the name (First, Middle, Last), rather than just next to the word 'Name', like it does when using the Name field. All in all, a much more complicated process than it needs to be.

  7. @alsc: Thanks for the help, but those aren't really what I'm looking for. I understand that we have the CSS ready classes, that we can have multiple columns in one row, etc. Really, all I'm asking for is that GF make it user-friendly to have a middle name field optional when adding a Name field.

  8. I'll second what Philip is asking for. Middle names are quite commonly needed for things such as employment applications, loan applications, and the like – much more so than the Prefix and Suffix fields that are already part of the "extended" name field.

    While I can create this on my own using three separate fields and CSS ready classes, that's an awfully long way to go for something that should be a simple checkbox when setting up the form.

  9. Would love some update on this-- Gravity forms is a fantastic product, but this incredibly tiny oversight is really annoying. Please add a Middle Name option to the name field as soon as possible.

    It is best if you all just continue to build out forms with single line inputs if you need this as we haven't made this change as of yet. The team does check feature requests often for ideas and updates.

    Well I just bought the Gravity Forms plug-in for the primary purpose of creating an online rental application from my paper applications...based upon the claims of how easy Gravity Forms are to use. The first fields I need to enter are the First, Middle and Last names...very preferably on one line. From reading the thread above, it looks like that is not an easy option. Middle names or at least initials are critical in this application because they help to differentiate the individual from others with the same first and last names, which is a common occurrence when doing a background search in the public records. I hope this will become a priority sometime soon?
