Puerto Rico can only currently be used, from my understanding, as a country, and not as a state as part of the United States, as well as other US commonwealths. Would it be of much difficulty to add the ability to include these other areas as part of the US in terms of state listings, so that we don't have to include an entire international address field for only one location?

Allow Puerto Rico as a state

You can create your own address type with this filter.. that would give you the flexibility to set it up however you'd like it.
