    How do I integrate approval on a user registration form before the user becomes active?

    Moderation is not a feature of the User Registration add-on but it is offered by other WordPress plugins:

    One example:
    http://wordpress.org/extend/plugins/theme-my-login/

    Another approach would be to assign a role with little capability, then change the role after you have 'approved' them.
