I am looking at purchasing this plugin as it is the only plugin that will integrate with my affiliate software. My question is, can I customize the form? For example I would like to change the submit button to an image. maybe add an image in the form. Here is an example of what I want it to look like.

Yep, there is an option to use your own image in place of the standard button for the form.

That's exactly what I was looking for.. I'm assuming I can also change the background color and add HTML inside the form so I can add text and images too?
My next question is do I need to buy the business package to get my forms to integrate with Aweber? Or will I be OK with just the personal package?

You can style the forms completely with CSS in your theme's style.css. There are HTML blocks you can add to your form.
To use the Gravity Forms Aweber Add-on, yes you need at least a business license. The basic add-ons are not available to personal license holders.
