Creating Custom Password Reset Form

How do you see this working?

I don't think you would need the User Registration add-on for this. You could use a regular Gravity Form. You'd have to consider who you let access the form and what sort of verification you perform, but in your form you could generate a new password for them, or allow them to set it, then update the usermeta record. It's possible.
