Custom Madlib style.

    I have setup a basic Madlib style form using the support tutorial however I need to have a few changes and I don't have the time to complete the work myself. I really need it to look like the following http://www.howto-get-socialsecurity-disability.com/form.jpg

    I need the following changes.
    First I need it to be styled like the one in the jpg. the layout is exactly what we are looking for.

    Second, This one is a must have. I need the field titles to appear inside the field like in the jpg. So people will know what is expected of them. I currently have the description over top of the field but we do not like this. It must also disappear when you click in the field, I tried to use default value but this was confusing to our test group.

    Let me know how much and how long it will take.
