for our website. I believe it is a Gravity Form. We want a visitor to fill out a questionnaire, submit the form and based on their answers, get a custom submission response. What version do we need to purchase to get this type of functionality?

You can accomplish a conditional response with any Gravity Forms license. If you have just one site and no need for any of our add-ons, you can start with the personal license. You can upgrade at any time and will be credited the price you paid for your existing license.
There are posts in the support forums which we can point you to to show how this has been done in the past.
