Features needed before buying the plugin

    I'd like to ask if the plugin can support these features:

    The plugin must be able to create the following:
    1) customised the form
    2) send notification email to webmaster on new submission
    3) output format in excel/csv
    4) output file can be individual customer submission as well as a compilation of all customers record.

    Lets take this one at a time.

    1. You can add as many or as few fields as you like, from basic text and number fields to more advanced multi-column fields that allow the user to dynamically add additional rows as they fill out your form. You can customise it's appearance using css ready classes which allows to you position fields into columns and using your own custom css in your themes stylesheet you can customise it's appearance even further.
    2. Your form can be configured to send an admin notification to the email address of your choice and if you want to can also enable user notifications as well. For email notifications to work however your webserver must be configured to send email or you can install a SMTP plugin.
    3. Gravity forms supports exporting entries in csv format
    4. I can't remember if you can export an individual entry but there is definitely an option to export a date range
