Form Format Clean Up/ Quick Design by a good eye

Hello,

I am looking for someone that could quickly tidy up the following pages. I have limited to no knowldege of CSS and in a time crunch. (no time to learn although its simple from what i have read). Sitiation, once I remove the sites style sheet. All fields jam together. Just need someone who could space every thing out to look estheically pleasing. Quick job but there is a an urgency. Once complete, tagging fields to an SQL database to follow in a week or so if interested.
