Description: We are re-developing a site for an client with Word Press and Gravity Forms.
There is an old form for equipment rentals we made years ago that we would like to re-make with gravity forms. You can review a sample of the form we need to recreate here:

We need to make sure the new form is secure and pci compliant as the client wants to capture credit card data.
Perhaps the best solution is to have the form calculate the total and make the charge via authorize.net so that the card data is not stored on the server.
Form submissions also will need to be downloaded in csv format.
