We need a gravity form open in light box (we use this template http://demos.brianmcculloh.com/swagger/ and he already have a light box installed)
This gravity form have to use dynamic data (URL of the page and post title)
It must have :
First Name, Last Name, Calendar period (arrival date, departure date), number of persons, email, phone number, and gsm number
The same form will be using on a lot of page.
we need to make this happen ASAP. I will need to know costs and timelines
