Project: User meta generated certificate for continuing education
I need someone to assist with creating an evaluation form that will generate a certificate at the end that will be emailed along with the option for the user to print out that pulled information from what they had chosen. It is for CE credits if that gives you a better idea of what I'm looking for. Here is an example of what a company has done before for us, on their own platform: EXAMPLE

This certificate would automatically pull the user's Name, Degree, calculate the credit hours based on what courses they selected that they attended, as well as print out on a separate page, everything they choose after the certificate. Please email me with timeframe and estimate.
