Horizontal Radio Buttons?

    I am doing a survey form with a 1 to 5 rating scale. SurveyMonkey does this was a set of horizontal radio buttons. Is there a way to do this with Gravity?

    Hi, please excuse my lack of knowledge
    I have looked at the link:

    downloaded the files gf_likert.js & gf_likert.css, but I don't know where to put them and where to add the other line of code - it says add it to the header.php, but I don't know where abouts to add it.
    I've edited a lot of php files before, but only when I know exactly where to put the code.
    I really like the look of the radio buttons in this link - if anyone can help, it would be much appreciated.
