Yes, Gravity Forms could handle a similar form. There would most likely be some formatting differences, but the section breaks and field types are standard fields available in GF.
As a quick follow up note.. Gravity Forms doesn't provide the security for the form, you'd have to host it on a secure web space if you wanted to take credit card information, etc.
Kevin, the SSL is not an issue on our end, but the complexity was a concern :-)
My next question is can a value somehow be passed to pre-populate a field?
My last question, for now, is do you know of any conflicts with EasyVideoPlayer? We tested cforms II and it kept videos from loading which is a deal killer.

Yes, you can pass values via querystring parameters or function hooks to each field.
I'm not aware of any conflicts with the EasyVideoPlayer plugin. Can you post a link to the plugin for me? I'll give it a try and let you know what I find.
Kevin, EVP is actually a web application that simplifies the process of hosting video on Amazon's S3 service. The embedded videos in wordpress posts/pages simply stopped working when we started testing cfroms II and started working once we deinstalled the plugin. If you've not heard anything as of yet then we'll just see what happens if we purchase your plugin.
