Can you create an order form that has a set-up cost (initial payment) and also a recurring (subscription) payment.

eg. $110 set-up cost (which is actually $60 set-up cost + $50 first month) and $50 per month there after.

    Total: $110.00
    + $50.00 p/m after

  2. How you do it depends on the payment add-on you're using. Which one are you planning on using?

  4. This is not a feature of the PayPal Payments standard add-on, but there is a 3rd party add-on which might help: http://wordpress.org/extend/plugins/gravity-forms-subscriptions/ That was not created by us so I'm not familiar with how it works.

Our PayPal Payments Pro add-on supports a trial period, which could be used for your setup fee and first month's payment. Then you can have a recurring subscription of your normal monthly amount.
