Multiple Choice List - Create Text Entry Box Last

    Is there a way to add a text entry box for user input into a multiple choice list? In other words, choice 5 does not have a predetermined number and users enters an amount:

    choice 1 $10
    choice 2 $20
    choice 3 $30
    choice 4 $50
    choice 5 $ Text Box (user can enter their own amount here)

    Choices 1-5 are still mutually exclusive, as a single list.

    No, this isn't an option. What you can do is use conditional logic to show a text input box after the multiple choice field if the user selects the last option.
