Need form with conditional questions, math function and a slider

  1. Will Gravity Forms right out of the box help me create the following form?}
    1. Which picture do you like? Click one.
    2. How many people will be joining you? (1-7 pull down or click on image)
    3. Cost per person: (Package 1: $XXX/the answer to question 2; Package 2: $XXXX/by the answer to question 2, etc. as a Pull down menu.)
    4. Where do you see yourself on this spectrum? (Slider)

    It would be great if the survey form had a lot of eye candy built in. Suggestions?

  2. Out of the box: not everything.

    1. Yes, you can use an image to replace a radio button to record their choice.

    2. Yes, with a pull down. I'm not sure how an image would be used here.

    3. Yes.

    4. Sliders are not built in and I don't have an example of anyone replacing a number input with a slider in Gravity Forms, although I know there are lots of jQuery number slider plugins. I've just not seen one implemented yet.

    Eye candy is all up to you. Gravity Forms supports an HTML field, you can add images to your form, and you can also style your form to look however you like, using CSS and your child theme's stylesheet.
