I need to make an order form with multiple fields. I need to be able to export the form in two ways, one with just the image numbers as a csv file and another sorted by number and as a txt or excel file, oreferably a txt file. It needs to look like this:
Image number B&W Must Have Notes:
The b&w and must have fields can be check boxes. When sorted each row needs to stay together. So 4 columns and an unknown number of rows. Ideally a line number would appear on the left of each row. Probably no more than 100 rows I would guess.
I would also need to collect client information on the form.
Is this something I can do with your forms?
