And does it export to Excel?

You should be able to easily reproduce the form you referenced with Gravity Forms. You could even use the conditional logic feature to only show the "if yes" fields when then yes option is actually selected. That would make the form more compact.
To answer your second question, yes, you can export your form entries to CSV format for use in Excel or other spreadsheet programs.

When you mention "conditional logic" that makes me wonder. Is the form development interface mostly a WYSIWYG type program with obvlious selectors to control the conditional logic options?

The form builder is a simple visual interface with "drag & drop" field ordering, easy to understand options, etc. The conditional logic operations are pretty simple to pick up.. once you set up your fields, you have the option to show/hide fields or complete sections based on the value of another. You shouldn't need any programming experience to do most normal forms. Gravity Forms does have some advanced functionality that those with programming experience can expand upon if desired though.

As a super satisfied Gravity Forms user, I can confidently say you can easily recreate that form using Gravity Forms. I don't see anything particularly complex about it at all, and the form you create can be even more compact, as Kevin stated. The forums here are great so if you have any trouble creating your form, posting your questions here will get you quick, helpful answers.
