(Why is Input Mask for Phone enabled for Single Line Text and not the designated Phone “Advanced” Field?)
Also having the pop-up calendar embedded into the form with the selected date highlighted would be nice.

All of these features, except for the phone number formatting are related to the JQuery calendar we use: http://jqueryui.com/demos/datepicker/ You can extensively customize this date picking solution to have most, if not all of what you want. We didn't set a UI for this because those that want it can easily customize it, and with all the options we do provide, we didn't want to clutter or confuse users even more.
