Is it possible to add a question type with 'multiple row' text entries? It would work like the Likert scale, such that you can add multiple rows such as 'First name', 'Last name', 'Streetname', 'Postcode' etc... but in a single survey-type question, rather than spread the different text inputs over 4 different questions.

You could maybe use the list field to do what you want with four columns? That would be as close as you could come currently.
