2 radio lists per field? 3 column layout?

    The survey above is styled how I would like it to look, but I got there by having the statements be HTML blocks, the two radio lists being separate fields/questions, and putting each line in a 3 column layout. I put field labels on the first two radio lists, but deleted them from subsequent ones to get the column title look.

    The problem is I'm not able to decipher what answer goes to what question once the data is submitted. I thought removing the field labels but designating Admin labels would take care of this, but it doesn't show up this way in the email.

    Is there a better way to achieve this look and have the survey answers be properly organized and labeled?
