    Is there a way to create four columns? I need more than three columns for one of my forms and I am having trouble creating my own.

    Do you have a link to your form and can you explain a little more why you need 4 columns and what types of fields you need in those columns?

    Yes, the non profit I volunteer for wants four columns under "Sources of Income". The "Name" field will span four columns and so on.
