A gravity form that was previously working fine will no longer show up.

  1. A gravity form on our website located at http://www.parkvillebni.com/presentation-bios/ is no longer visible on the page or in the preview on the admin side. The form has not been edited and the shortcode has not been changed from [gravityform id=5 title=false description=false ajax=true]

    Any ideas why it would suddenly disappear and not even show up in the form preview? It used to work just fine.

    We have updated the plugin to the latest version and are using wordpress 3.5

    This is most likely a theme or plugin conflict. Can you please follow these instructions to isolate the issue?

  3. I will, but 5 other forms work just fine, it is just this single form that is broken. I'll report back after I do this.

    It looks like you have the following JQuery version in the header of your site that would stop conditional logic from working:
