"Add Another" button that creates another group of fields for each item

Is there a way to create an "Add another child" button that will dynamically create another page or list of fields that they fill out, including things like name of child, age, class they want to take (radio buttons), etc. ? I know there is a list feature, but from what I can tell, it only allows text fields, not radio buttons, etc. for column values.
