Add Form selector on wp page is empty

    Dashboard --> Forms shows 2 forms. However is I go to a page and try to add one of these forms the "Select a form" drop down menu is empty. Any ideas what I am doing wrong?

    What versions of WordPress and Gravity Forms are you running?

    Can you try testing for a plugin/theme conflict
