Add form submission to the middle of info in email notifications.

I'm trying to find how to do this but can't. However, I know GF can do this because it's fairly simple.

I need to make a field that's filled out with simple info, but when the notification email goes out, that particular field entry will appear in the middle of a URL or other information. Examples below:

Input into a field: 9820

Shows up in the email notifications as http://www.mywebaddress.com/affiliates/wmp.php?id=9820

And I need to also know how to make it appear like this: Your ID is 9820 to be used with....
