Add new form - coming up blank

    Hello,
    I currently have 3 forms using GF 1.5.2.8 I have tried to add a fourth and received a blank screen showing only the save form button.

    I have tried to troubleshoot the issue by:

    • Deactivating all additional plugins
    • Un installing GF and removing the db tables. Then reinstalling

    These steps did not work. Any other ideas? The website in question is my development site http://trimarkassociates.net

    This problem has been resolved.

    The root cause were some server based errors:
    Pureftpd - Built in limitation for number of files uploaded per user

    Nginx - Automatic DDoS protection for connection between Nginx and Apache

    I'm posting this in the case others have similar problems and to provide some troubleshooting clues.
