Auto fill a field from the post_title of {referer}

Hi,

I have created a form wich is at the end of a custom post content.
What I want is to Pre-fill two fields with informations from the content (TITLE and URL)
I used the merge tag {referer} to auto populate the field of the post URL but I can't find a way to
auto populate a field with the post_title (of the content from wich I come)
Is it possible ?
