Auto-fill Address City and Zip

I am trying to have my contact form automatically fill in pertinent data when the user starts entering it. Example: When I enter the first letter of my name, my full name appears. If I tab out of the field—I am using Safari—the name stays and any of the fields that match my contact data are filled in throughout the form. The trouble I am having is that I get the street address twice. The second one appears in the City field. Also, no zip is filled in. How do I get those to work?
